What VO2 max is
The maximum volume of oxygen your body can take in and use per minute, per kilogram of body weight — millilitres of O₂ per kg per minute. It's the ceiling on your aerobic engine, set by how much blood your heart can pump and how well your muscles extract oxygen from it. Among fitness measures it has an unusually strong relationship with how long people live, and unlike most risk factors it's directly trainable.
The three methods, and what each costs you

VO2max ≈ 15.3 × (HRmax ÷ HRrest). Requires no exercise whatsoever, which is remarkable, and is the least accurate of the three. It was validated in well-trained men; for everyone else the error is wide. Treat it as a ballpark. - Cooper 12-minute run — Cooper, JAMA 1968.
VO2max = (metres − 504.9) ÷ 44.73. Run as far as you can in twelve minutes. Simple, well-validated, and requires genuine maximal effort and even pacing to mean anything. - Rockport 1-mile walk — Kline et al., Med Sci Sports Exerc 1987. Walk a mile fast, take your heart rate at the finish. Designed for people for whom a maximal run is unsafe or unrealistic, and the best choice if you're older, deconditioned, or carrying an injury.

Worked example
A 30-year-old man with a resting heart rate of 60. Tanaka puts his max HR at 208 − 0.7 × 30 = 187. Uth then gives 15.3 × (187 ÷ 60) = 47.7 ml/kg/min — about 13.6 METs, and "Good" for his age. If he instead ran 2,400 m in the Cooper test, that's (2400 − 504.9) ÷ 44.73 = 42.4. A 5-point gap between two methods for the same person is entirely typical, and is the honest error bar on all of this.
About the fitness bands
The bands are anchored to Cooper Institute percentile norms for ages 20–29 and aged forward at the well-documented decline of roughly 10% per decade in untrained adults, rather than looked up per decade. They're for orientation — a rough sense of Poor through Superior — not a precise rank against a real cohort. The estimate feeding them already carries several points of uncertainty, so don't read much into landing near a boundary.
What moves it
VO2 max responds to training faster than almost any other physiological measure — 10–20% gains over a few months are ordinary in someone starting from untrained, and high-intensity intervals raise it faster than steady-state work alone. It falls roughly 10% per decade if you do nothing, and considerably less than that if you keep training. Losing fat also raises it mechanically, since the number is per kilogram of body weight.
FAQ
Why does my watch give a different number?
Watches estimate VO2 max from the relationship between your pace and your heart rate during outdoor runs, which is a fourth method again — usually a licensed Firstbeat model. It's often the best of the estimates because it draws on many sessions rather than one test, but it needs GPS runs at varied intensities to calibrate, and it inherits any error in your max heart rate setting. A gap of 5 points between any two methods is normal.
Which method should I use?
Cooper if you can safely run flat out for twelve minutes and want the most defensible number. Rockport if you can't. The resting-heart-rate method if you want a free ballpark today with no effort — just don't take it too seriously, and don't compare it against a Cooper result and conclude you got fitter.
What's a good VO2 max?
Context matters more than the raw figure. Roughly: below 30 is poor for most adults under 50; the mid-30s to low-40s is typical for an active person; above 50 means you're training seriously. Elite endurance athletes reach 70–85, and the highest reliably recorded values — in cross-country skiers and cyclists — sit around 90. Sedentary older adults can drop below 20, which is the level at which everyday tasks start to get hard.
Is the 12-minute test safe?
It's a maximal effort, so the usual caveats apply: it's a bad idea if you have known heart disease, uncontrolled blood pressure, or symptoms during exercise, and if you're over 40 and untrained it's worth clearing with a doctor first. The Rockport walk exists precisely because a maximal run isn't appropriate for everyone.
Why is it per kilogram?
Because moving your own body is what aerobic capacity is usually for. Absolute VO2 max in litres per minute favours large people; dividing by body mass makes the number comparable and reflects what you experience running uphill. It also means losing 5 kg of fat raises your VO2 max without your heart or lungs changing at all.
